b. Use the five key determinant areas to organize SDOH profile information for your community/geographical area.
4. Select and examine a health care topic to analyze within the context of the selected community of focus. For example, after completing the community-level health profile, you may choose to examine the prevalence of obesity within the community.
5. Perform a windshield survey of your selected community and focus your survey activity to include your health care topic. Complete an asset/gap map of resources.
